Licensed Social Worker
Essential Job Functions / Responsibilities
Assesses psychosocial status of patients and families/caregivers related to terminal illness, communicates findings to RN and interdisciplinary team, provides assessment in patient residence and assistance plan.
Performs social evaluations of family dynamics, caregiver abilities, communication patterns, suicide risk, neglect, abuse, plans intervention based on findings.
Provides counseling to patients and families/caregivers as needed, offers crisis intervention when necessary.
Assesses cultural needs of patient and family, educates interdisciplinary team on cultural considerations.
Works under supervision of MSW.
Maintains clinical records for all referred patients.
Educates patients/families, assists in advanced directive preparation.
Provides information and referral services regarding practical and environmental needs.
Serves as liaison to community agencies, maintains contracts with public/private agencies.
Participates in development of individualized plan of care, attends interdisciplinary meetings, assists team in recognizing psychosocial stresses.
Assists physician and team members in understanding social/emotional factors related to health problems and dying issues.
Assists Bereavement leadership in program to meet needs of families/caregivers 13 months post death, complies with regulations.
Ensures initial bereavement assessment assigned timely.
Assists Chaplain in condolence call.
Maintains bereavement support group schedule.
Assists family/patient in planning funeral arrangements, financial, legal, health decision responsibilities.
Maintains compliance with HIPAA, treats patient info as confidential, uses protected health info on minimum necessary basis.
Additional Duties
Attends interdisciplinary group meetings.
Assumes responsibility for personal growth and development, maintains and upgrades professional knowledge through continuing education.
Actively participates in quality assessment and performance improvement teams and activities as assigned.
Other duties as assigned.

Qualifications
Bachelor’s degree in social work from an institution accredited by the Council on Social Work Education; Master’s degree preferred; licensed in the state served; one year of social work experience in a health care setting.
Minimum of one year of experience in health care; hospice experience preferred.
Good verbal and written communication, organization skills.
Current CPR Certification.
Ability to pass DHS background study.
Valid State Driver’s License, automobile in good working order, insurance in accordance with state requirements, acceptable driving record if driving required.
