Cleveland Clinic . This position supports the South Team covering mostly Summit and Medina counties (Medina, Brunswick, Wadsworth, Lodi, Stow, Akron, Cuyahoga Falls, Hudson, Fairlawn and Macedonia). You will provide high quality nursing care to patients in their homes, including assessments, complex nursing treatments, and education to help avoid readmissions to the acute care setting. After visits, Home Health RNs complete documentation remotely, coordinate patient care, and arrange visit schedules. Cleveland Clinic Home Care provides a tailored orientation based on experience. This position is eligible for student loan repayment. Caregivers in this role work 8:00am to 5:00pm or 8:00am to 6:00pm with options for 8- or 10-hour shifts. On-call, weekend, and holiday requirements apply. Responsibilities
Provide direct nursing care. Perform accurate and complete admission assessments and reassessments within a specified time frame. Assess patients for safety risks, abuse and neglect. Serve as a primary coordinator of all disciplines for well-coordinated patient care. Assess, document, prioritize and manage patients’ pain. Minimum qualifications
Graduate from an accredited school of Professional Nursing Current state licensure as a Registered Nurse (RN) Basic Life Support (BLS) Certification through the American Heart Association (AHA) Valid Ohio Driver's license upon hire and per policy 8-175 thereafter Proof of $100,000/$300,000 automobile insurance coverage upon hire and per policy 8-175 thereafter One year of current experience as an RN One year of experience in an acute Medical/Surgical or Home Care setting Preferred qualifications
Bachelor’s of Science in Nursing (BSN) Familiarity with complex medical treatments Experience working in homecare, med/surg, ICU and/or ED areas Benefits and culture

Requires full range of motion, manual and finger dexterity, and eye-hand coordination Requires standing and walking for extended periods Requires corrected hearing and vision to normal range Exposure to communicable diseases or bodily fluids Medium work: exerting 20 to 50 lbs occasionally; 10 to 25 lbs frequently; up to 10 lbs constantly Must be ambulatory and able to drive to patients\' homes, climb stairs, and assist movement of patients Must be able to read, write and communicate with patients Personal protective equipment
