Overview
As a Home Health Physical Therapist with CenterWell Home Health, you’ll play a vital role in helping patients regain strength, mobility and independence — all from the comfort of patients’ homes. You’ll deliver personalized care that focuses on rehabilitation and functional improvement, empowering individuals to overcome physical limitations and enjoy a better quality of life. Base pay range: $85,400.00/yr - $117,500.00/yr. Pay range may vary by geographic location and experience. Your actual pay will be based on your skills and experience. Responsibilities
Plan and administer prescribed skilled physical therapy treatment and training for patients with injuries, illnesses, or functional disabilities to attain the highest level of physical function. Test and screen patients’ physical strengths to support physician evaluation and develop/measur e treatment programs with measurable objectives. Develop and implement a conditioning/rehabilitation program aligned with the physician’s Plan of Treatment and patient/rehab team goals; adjust treatment as needed. Collaborate with physicians and clinical team members to obtain patient information and revise therapy plans as necessary. Provide guidance and supervision to Physical Therapy Assistants and Home Health Aides; ensure care plans reflect current therapy needs and monitor documentation completion. Document care observations, interventions, and evaluations promptly and accurately; process interim orders and ensure timely physician signature and implementation. Report patient progress to the physician, Clinical Manager, staff, patient and family; submit evaluations, treatment plans, progress reports and discharge summaries as required. Required Experience / Skills
Degree from an accredited Physical Therapy Program (APTA approved) Minimum of one year physical therapy experience preferred Current and unrestricted Physical Therapy license Current CPR certification Strong organizational and communication skills A valid driver’s license, auto insurance, and reliable transportation Pay Details
