Located in sunny Fort Myers, FL, Shell Point offers state‑of‑the‑art rehabilitation facilities, a collaborative interdisciplinary team, and a vibrant, engaged resident population.
Sign‑On Bonus Shell Point offers a $15,000 sign‑on bonus. Only direct applicants qualify for the bonus.
Job Summary Screen, evaluate, re‑evaluate, discharge plan, and make revisions in any established plan of care under physician supervision.
Responsibilities
Screen and evaluate patients upon admission.
Re‑evaluate patients and modify plans of care as needed.
Discharge patients or transition them to appropriate levels of care.
Collaborate with physicians, nurses, and other interdisciplinary team members.
Prepare accurate documentation and maintain patient records.
Supervise Physical Therapy Assistants as required.
Qualifications
Bachelor’s, Master’s, or Doctorate in Physical Therapy from an accredited program.
Active state license and certification as required by state and national board associations.
Excellent organizational and time management skills.
Strong written and verbal communication skills.
Working knowledge of Functional Pathways policies and procedures.
Ability to supervise Physical Therapy Assistants.
Completion of all required screenings, certifications, and licensure.
