We are seeking a compassionate and experienced

Psychiatric Registered Nurse (RN)

to join our Home Health team. This full-time, hands-on role is designed for dedicated professionals who are passionate about supporting patients with behavioral health conditions in the comfort of their homes. The Behavioral Health RN will deliver tailored interventions, medication management, and clinical support to help patients live more stable, fulfilling lives. Key Responsibilities

Conduct comprehensive patient assessments to diagnose psychiatric and behavioral health conditions. Develop, implement, and update individualized treatment plans in collaboration with interdisciplinary team members. Provide medication management, patient education, and adherence monitoring. Perform regular home visits to assess progress and adjust care plans. Document care accurately in compliance with legal, ethical, and organizational standards. Educate and collaborate with families and caregivers on condition management and care needs. Respond to crises, stabilize patients, and use therapeutic communication to ensure safety. Maintain patient confidentiality in accordance with HIPAA regulations. Engage in continuous professional development to remain current in behavioral health best practices. Advocate for patients to ensure access to appropriate healthcare resources. Requirements

Active RN license in the state of practice. Minimum of 2 years experience in behavioral or mental health nursing. Recent experience in acute psychiatric, outpatient, or home health psychiatric settings preferred. Experience with Dementia/Alzheimer's patients strongly desired. Prior home health experience highly valued. Advanced knowledge of psychiatric nursing principles and protocols. Ability to work independently and manage a caseload effectively. Proficiency with EHR systems and clinical documentation. Current CPR certification. Valid driver's license and reliable transportation for home visits. Flexible schedule, including evening or weekend availability when needed. What We Offer
