We are looking for experienced Licensed Nurse Practitioners, NPs, General Medicine Nurse Practitioners for “SNF-ists,” in‑house nurse practitioners that will provide care for the geriatric population in a Skilled Nursing and/or Assisted Living setting.
What You Will Do
Act as a Skilled Nursing Facility’s leading provider of internal medicine services.
Collaborate with the facility’s interdisciplinary team and physicians.
Serve as a day‑to‑day clinical leader while helping oversee the care of all the facility’s residents.
Work with the entire facility’s patient population—short and long‑term residents—overseeing their day‑to‑day care with the assistance of the facility’s nursing staff and guidance from medical directors and attending physicians.
Provide comprehensive health assessments, differential diagnoses, and prescribe treatments under the protocol in the direct management of acute and chronic illnesses and diseases.
Prescribe all necessary medications and treatments.
Perform comprehensive physical examinations of patients.
Develop and implement patient management policies and procedures.
Facilitate referrals to other healthcare professionals and medical facilities.
Communicate with attending physicians and/or specialists regarding patient care.
Be comfortable and proficient with documentation and CPT coding.
Conduct monthly stability visits for long‑term residents.
Perform evaluations for patients exhibiting changes in condition.
Engage in Advanced Care Planning.
What You Will Need
Licensure: Advanced Practice Licensure (APN) with an applicable state license.
Active license and DEA number.
Minimum one‑year experience in a Skilled Nursing setting (preferred).
Ability to build rapport with patients and their loved ones.
Ability to thrive in a fast‑paced environment.
Excellent written and verbal communication skills.
Strong leadership experience with emphasis on teamwork in a healthcare setting.
Master’s Degree required.
W2, pay‑per‑encounter or fee‑for‑service role.
Education: Master’s Degree Required.
