Physical Therapist – Full Time Granite Creek Health and Rehabilitation in Prescott, AZ is seeking a passionate and dedicated full‑time Physical Therapist (PT) to join an in‑house therapy team. This role offers a supportive environment, clinical development, and career growth.

About You

Licensed Physical Therapist able to work with diverse patient population at all experience levels.

Passionate about positively impacting patients’ lives and helping them regain independence.

Committed to evidence‑based treatments and collaborating with interdisciplinary team.

Responsibilities

Assess, evaluate, and develop treatment plans for patients with cognitive and/or ADL physical functioning disorders.

Administer rehabilitation interventions including ADL retraining, therapeutic exercises, modalities, and assistive devices.

Regularly assess patient progress, adjusting treatments accordingly, and communicate updates with the healthcare team.

Provide counseling and consultation to patients, families, caregivers, and service providers.

Document treatment plans and outcomes in line with regulatory and clinical practice standards.

Contribute to patient‑centered care by evaluating treatment effectiveness and making necessary referrals.
