Care Team Assistant II – Dallas, TX
LifeSpan Home Health is a trusted and established home health agency committed to helping elderly individuals and people with disabilities live safely and comfortably in their homes. We provide medical and non‑medical care with a focus on dignity, independence, and community.
What You’ll Do:
Receive and process home care referrals.
Schedule visits based on the plan of care.
Act as a communication hub between physicians, case managers, staff, and clients.
Educate inquirers about services and home‑care options.
Reschedule visits as needed and ensure accurate time records.
Conduct client admissions/evaluations (excluding PHC program).
Submit timely reports and collaborate with payroll.
What We’re Looking For:
High school diploma or GED (required).
6 months to 1 year of experience in a home health or medical setting (preferred).
Willingness to travel for client visits/hourly care.
Strong communication and organizational skills.
Bilingual in Spanish (preferred).
A heart for service and a commitment to quality care.
Must have reliable transportation, valid driver’s license & auto liability insurance**.
