About PACE Philanthropists Gary and Mary West founded West Health with the mission of enabling seniors to successfully age in place.

The Gary and Mary West PACE (Program of All‑inclusive Care for the Elderly) is a non‑profit delivering medical, social, and behavioral services to frail elderly in North County San Diego.

Position Summary Under the direction of the Center Director, the Care Navigator supports the interdisciplinary team (IDT) by coordinating the Service Delivery Request process and documenting meetings. As a member of the Retention Committee, the Care Navigator provides direct assistance to participants, proactively resolves concerns, and works to prevent disenrollment.

Essential Functions

Participate in IDT meetings and provide support.

Scribe IDT meeting agenda and assist with coordination.

Coordinate transportation schedule in IDT and track cancellations.

Manage Service Delivery Requests, including EMR initiation, documentation, NOA letters for denials, and service dates.

Collaborate with medical assistant scheduler for assessment and transportation scheduling.

Lead Retention Committee and liaise between disciplines, services, and participants to ensure continuity.

Interface directly with participants at risk for disenrollment.

Clarify needs, resources, and develop plans with IDT and participants/families.

Facilitate communication among service providers, family, participants, and caregivers.

Maintain courteous, helpful, and professional behavior.

Promote a collaborative work environment.

Other Functions

Special projects, including implementation of satisfaction surveys.

Consistently contribute to problem solving and cooperate with identified resolutions.

Present process improvement ideas to supervisor.

Maintain verbal and written skills required for the position.

Attend meetings and participate in committees as required.

Adhere to all GMWP policies, safety policies, and OSHA guidelines.

Communicate clearly and concisely with sensitivity to others.

Education and Certification

High school diploma or GED equivalent required.

Bachelor’s Degree in health or human services preferred.

Skills & Experience

Two years of health care experience with frail and/or chronically ill elderly.

Knowledge of community resources for seniors and families.

Experience providing case management, including motivational/clinical interviewing.

Excellent written and verbal communication skills; able to interact clearly with internal and external customers.

Pass pre‑employment drug/alcohol screening and criminal background check.

Current California Driver’s License with compliant driving record; maintain automobile insurance.

Proficient in Microsoft Word and Excel.

CRM and EMR experience preferred.

Skilled in building relationships with participants, co‑workers, vendors, medical staff, and the public.

Medical Clearance Employees must have medical clearance for communicable diseases and up‑to‑date immunizations before direct participant contact.
