Physical Therapist – BoldAge PACE
BoldAge PACE provides comprehensive physical therapy services to elderly participants, focusing on assessment, treatment, and education to improve independence and quality of life. The Physical Therapist works collaboratively with an interdisciplinary team, performs home safety evaluations, recommends adaptive equipment, and documents progress.
Job Summary Plans, organizes, and implements Physical Therapy services for PACE participants. Responsibilities include assessment, planning, providing treatment, and teaching participants, caregivers, or family to maintain participant support in the community.
Essential Duties and Responsibilities
Provides high‑quality care and serves as a member of the PACE interdisciplinary team (IDT).
Performs in‑person assessments for participants at initial enrollment and as‑needed ongoing assessments.
Assesses the need for adaptive durable medical equipment, specialized shoes, and prosthesis; evaluates participant home safety and recommends home modifications.
Plays for and administers physical therapy treatment programs in accordance with recognized standards and policies.
Documents evaluation, assessment, plan, and treatment rendered accurately and timely according to policy.
Measures effectiveness of treatment, documents and revises plan accordingly.
Educates and instructs participants/family in procedures, home programs, and confirms understanding.
Trains PACE staff in body mechanics, use of equipment, transfers, gait patterns, and safety techniques.
Provides care in the center, clinic, contracted facilities, and participants’ homes according to each participant’s plan of care.
Fosters positive relationships and communication with co‑workers, participants and families, and community members.
Collaborates with IDT to develop comprehensive care plans and participates in quality‑improvement projects.
Attends interdisciplinary team meetings, communications, and organizational meetings related to participant care.
Maintains confidentiality, follows policies, OSHA safety guidelines, and standard precautions.
Participates in continuing education and maintains relevant certifications.
Performs any additional duties as assigned.
Experience, Education and Certifications
Advanced degree in Physical Therapy from an accredited school.
Current license to practice Physical Therapy in the state of employment.
Current driver’s license and proof of auto insurance.
Two (2) years of experience on an interdisciplinary team in a hospital, nursing home, or community‑based setting.
One year of experience working with a frail or elderly population preferred; training will be provided if not present.
