We are seeking a dedicated and compassionate Registered Nurse with a Bachelor of Science in Nursing (BSN) to join our team as a Care Manager, Telephonic Nurse 2. In this pivotal role, you will play a crucial part in our Patient Management Program, focusing on individuals with specialized and complex health conditions, including autoimmune disorders, pulmonary diseases, neuromuscular disorders, infectious diseases, cancers, and other rare ailments.
Responsibilities
Assess and evaluate patients’ conditions via phone interactions, emphasizing medication therapy to ensure optimal wellness.
Develop and implement comprehensive care plans and monitor patient progress through regular assessments.
Provide medication education and use advanced telecommunication systems to communicate with patients.
Work independently while adhering to established protocols and guidelines.
Schedule & Hours
10 am–6:30 pm EST, three days a week Monday‑Friday.
11:30 am–8 pm EST, two days a week.
Friday late‑night rotations (11:30 am–8 pm) on a rotating basis.
Overtime required on an as‑needed basis.
Holiday rotation covering Martin Luther King Day, Memorial Day, Juneteenth, July 4th, Labor Day, day after Thanksgiving, and New Year’s Day.
Required Qualifications
Bachelors of Science in Nursing (BSN) – REQUIRED.
3–5 years of clinical acute‑care experience.
Licensed Registered Nurse in the appropriate state with no disciplinary action.
Must reside in Ohio, Kentucky, Florida, Arizona, or Texas.
Comprehensive knowledge of Microsoft Office (Word, Excel, Outlook).
Managed‑care experience.
Passion for improving consumer experiences.
Schedule adherence.
Preferred Qualifications
Experience with care‑management and adult patient education.
Auto‑dialer experience.
Bilingual English/Spanish.
Travel: This is a remote position; occasional travel to Humana's offices for training or meetings may be required.
Scheduled weekly hours: 40
Pay Range Pay range reflects a good faith estimate of starting base pay for full‑time (40 hrs per week) employment at the posting time. Pay may vary by location and by demonstrated skills, education, and experience.
$71,100 – $97,800 per year
Eligible for a bonus incentive plan based on company and/or individual performance.
