Job Overview
Housing Case Manager provides trauma‑informed services to program participants who often face multiple barriers such as mental health conditions, substance use disorders, chronic health issues, and frequent law‑enforcement contact. The role supports participants in achieving secure, stable, and permanent housing through assessment, documentation, goal setting, direct advocacy, and coordinated service referrals.
Key Responsibilities
Conduct trauma‑informed case management using motivational interviewing and strength‑based techniques.
Write clear case notes using the DAP protocol and update the Clarity HMIS and other case management systems.
Identify barriers and develop strategies to overcome them.
Screen individuals for program eligibility and complete needs assessments and program orientation.
Assess eligibility for permanent housing resources such as deposit/move‑in assistance, rapid rehousing, affordable housing, and permanent supportive housing.
Collaborate with Program Manager to utilize flexible client relocation funds equitably.
Link participants to services addressing barriers (mental health, substance use, health, legal, employment, education).
Build and maintain relationships with community providers and partners.
Support housing stability by working with property management, supervisors, and residents.
Manage conflict, de‑escalation, and crisis intervention, and refer to appropriate services.
Provide milieu management and support daily operations of the facility.
Develop and facilitate life‑skills training and workshops.
Partner with landlords to secure tenancy and assist participants through the move‑in process.
Follow up with participants after placement to ensure stability.
Participate in program meetings and corrective action planning as needed.
Assist in preparing monthly, quarterly, and annual reports, including HMIS reporting.

Required Knowledge, Skills & Abilities
Flexible crisis management and ability to handle fast‑paced environments.
Experience with case management, crisis counseling, advocacy, and referral services.
Knowledge of local resources for individuals experiencing homelessness or housing instability.
Proficiency in de‑escalation, crisis intervention, and stabilization techniques.
Strong record‑keeping and data entry skills, including HMIS and statistical reporting.
Excellent written and verbal communication; ability to communicate technical information effectively.
Ability to work independently and as part of a team, exercising sound judgement when unsupervised.
Knowledge of evidence‑based practices such as harm reduction, motivational interviewing, and strength‑based case management.
Demonstrated equity, intersectionality, and belonging awareness; ability to work with diverse populations.
Education & Experience
Minimum of two years’ experience in a social/human service setting related to housing and homelessness (required).
Experience and education demonstrating competency in equity, intersectionality, and belonging (essential).
Experience working in environments where language may be a barrier (desired).
Current Washington State Food Handlers Permit (desired). If not current, sponsorship available within one month of hire.
Valid WA driver’s license, insurance, and acceptable driving record for positions requiring driving duties.
A degree in human services, social work, or related field is a complement to the required experience.
Physical Demands
Regular requirement to talk, hear, stand, walk, use hands, lift and/or carry supplies up to 20 pounds.
Work requires long periods of sitting, walking, and occasional stair travel. Travel to program sites and meetings may be necessary.
Requires attention to detail, critical thinking, and the ability to participate verbally in meetings.
Work Environment
Human services environment in the Greater Seattle area with diverse client community.
Exposure to hazardous chemicals, blood‑borne pathogens, and other infectious materials—basic safety training will be provided.
No remote work; the position is on‑site only.
Facilities are monitored with camera surveillance for safety and security; cameras are used only in public areas.
