Location: Central New York Psychiatric Center's Five Points Residential Mental Health Unit on the grounds of Five Points Correctional Facility, Seneca County.
Pay Range Base pay range: $74,193.00/yr – $94,121.00/yr (NY State Office of Mental Health).
Duties
Provide crisis intervention, service linkages, and discharge planning.
Develop treatment plans with individuals and treatment team; monitor and evaluate progress; modify plans as needed.
Determine discharge needs and appropriate community resources.
Establish and maintain relationships with community providers to ensure continuity of care.
Minimum Qualifications
Licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW).
Considered under the Hiring for Emergency Limited Placement – Statewide (NY HELPS) program if licensed.
Within State Service: eligible for transfer to a competitive class position if have one year of permanent competitive or 55‑b/55‑c service in a title eligible for transfer under Section 70.1 of the Civil Service Law.
