Host Home Contract Monitor - Eastern Region
LSI is offering up to $500 sign‑on bonus for full‑time Host Home Contract Monitor positions in the Eastern Iowa region. Our Services for People with Disabilities is looking for a dedicated, energetic full‑time Contract Monitor for Host Home Services. This role encompasses the following counties: Allamakee, Benton, Black Hawk, Bremer, Buchanan, Butler, Cedar, Chickasaw, Clayton, Clinton, Delaware, Dubuque, Fayette, Floyd, Howard, Jackson, Johnson, Jones, Linn, Louisa, Muscatine, Scott, Winneshiek. May require travel outside of listed counties for service needs and meetings. Responsibilities Act as main contact for interdisciplinary team members and consumers in the sites. Work directly with consumers to provide support, assistance, and instruction in areas that facilitate personal growth and independence. Facilitate and participate in interviews and evaluations related to the contracting process for independent providers. Oversee contractual compliance and ensure assigned independent providers provide adequate care to consumers and meet contractual requirements. Participate in an on‑call rotation and may work long hours, including evenings and weekends, to ensure safety and satisfaction of those served. Qualifications Minimum qualifications: HS Diploma/GED and five years of related experience Associate’s Degree and three years of related experience Bachelor’s Degree and one year of related experience Preferred qualifications: Bachelor’s Degree in a human services related field and one year of related experience. Years of related experience in a supervisory or leadership role.
