Physical Therapist – BoldAge PACE The Physical Therapist provides assessment, treatment, and rehabilitation services to elderly participants in a community‑based PACE program, working closely with an interdisciplinary team to ensure individualized care plans.

Job Summary Plans, organizes, and implements Physical Therapy services for PACE participants. Responsibilities include assessment, planning, providing treatment, and teaching participants, caregivers, or other appropriate representatives to maintain community support.
Essential Duties and Responsibilities
Provides high‑quality care as a member of the PACE interdisciplinary team (IDT).
Performs in‑person assessments at initial enrollment and ongoing as needed.
Assesses need for adaptive durable medical equipment, specialized shoes, and prosthesis; recommends home modifications and equipment.
Plans and administers treatment programs consistent with recognized standards and policies.
Documents evaluation, assessment, plan, and treatment rendered accurately and in a timely manner.
Measures effectiveness of treatment, documents and revises plan accordingly.
Educates participants/family in procedures, home programs, and documents training and understanding by participant/family.
Trains PACE staff in body mechanics, use of equipment, transfers, gait patterns, and methods to maximize safety and functional abilities in performing activities of daily living (ADL’s).
Provides care and services in the center, clinic, contracted facilities, and participants’ homes according to each participant’s plan of care.
Assists participant to function at most independent level possible.
Develops and maintains positive relationships and communication with co‑workers, participants and families/significant others, and members of the community.
Collaborates with the interdisciplinary team to develop a comprehensive care plan for each participant.
Maintains regular attendance at and participates in daily Interdisciplinary Team meetings, communicates participant changes and collaborates with team members in care planning decisions and coordination for 24‑hour care delivery.
Acts as a quality improvement champion, participates in meetings, and implements best practices.
Fulfils the role of other IDT member(s) as needed according to licensure and competency requirements.
Participates in continuing education and maintains required certifications.
Other duties as assigned.
Experience, Education, and Certifications
Advanced degree in Physical Therapy from an accredited school.
Current license to practice Physical Therapy in the state.
Current driver’s license and proof of auto insurance.
Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home, or community‑based setting is preferable.
1 year of experience working with a frail or elderly population preferred; training will be provided if not present.
