Position Overview Pay range:
$35,570.00 - $57,800.00 / year Shift:
full-time AM Schedule:

Job Description Ensuring seamless patient care and efficient administrative operations. To support case managers and healthcare professionals by performing a variety of tasks that range from administrative duties to patient interactions. To facilitate and coordinate a safe and appropriate discharge plan while meeting the patient's individual needs as well as state and federal regulations. To work collaboratively with multiple other disciplines throughout the organization and community. To manage patient records and coordinate follow-up care. To enhance the patient experience and optimization of the overall healthcare delivery process. Qualifications Bachelor's degree in social work, psychology, sociology, or other healthcare related field. No experience required, 1-2 years preferred experience in healthcare setting (familiarity with healthcare or social service systems), such as in administrative or support role; Knowledge of community resources and health services preferred.
