PAY RANGE $75,000 - $125,000/year
JOB SUMMARY GrandCare Health Services is seeking a Physical Therapist (PT) to join our team. The PT is responsible for providing physical therapy services to patients according to a written physician’s plan of care.
COVERAGE AREA Long Beach • Signal Hill • Seal Beach • Rossmoor • Carson • West Carson • Lakewood • Lomita • Los Alamitos • Surrounding Areas
DUTIES AND RESPONSIBILITY
Assess and evaluate therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care.
Evaluate home environment.
Assess for muscle strength, mobility, gait, ROM—potential for rehab.
Direct physical therapy treatment.
Instruct patients and families/caregivers in the use and care of therapeutic appliances.
Determine priority needs for physical therapy.
Report to physician patient’s reaction to treatment or changes in condition.
Initiate physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient.
Instruct them as to the goals of the physical therapy program for the patient by participating in case conferences.
Train patient in the use of prosthetic device.
Identify patient and family/caregiver needs for other home health services.
Prepare and submit clinical and progress summaries based on the attainment of goals.
Participate in discharge planning for patient.
Provide physical therapy consultation to home families/caregivers.
Provide in‑service education programs for nursing organization personnel.
Participate in peer consultation process.
Supervise Physical Therapy Assistants according to organization policy and state regulations.
JOB REQUIREMENTS
CA Physical Therapy License
2+ Years of physical therapy experience, home health experience preferred
Demonstrate good verbal and written communication and organization skills
Possess and maintain current CPR certification
Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
