Speech Language Pathologist (SLP) – Hillcrest Village
Permanent, Full-time. CCCs and CFYs welcome to apply.
As a Speech Language Pathologist , you will develop meaningful relationships by learning about the life stories of our residents and staff. Each day, our SLPs utilize their clinical skills to promote the physical and psychosocial well‑being of residents, leading to a rewarding sense of fulfillment and satisfaction.

Key Responsibilities
Develop and implement individualized treatment plans based on comprehensive assessments.
Provide one‑to‑one and group therapy sessions, promoting physical and psychosocial well‑being.
Supervise and mentor Speech Language Pathologist Assistants and other clinical staff.
Participate in patient care conferences, utilization review meetings, and rehabilitation conferences.
Collaborate with nursing and other facility teams to ensure coordinated and comprehensive resident care.
Document assessments, treatment notes, progress, and recertification information in accordance with state and payer requirements.
Educate families and caregivers on maintenance programs and discharge planning.
Credentials & Qualifications
Current and valid Speech Language Pathologist (CCC or CFY) license in the state of Indiana.
Ability to conduct thorough assessments and accurately document changes in resident conditions.
Strong passion for geriatric nursing and commitment to senior care excellence.

Speech Language Pathologist – Long Term Acute Care Permanent.
Evaluate patients within twenty‑four (24) hours, or one business day of physician referral. If unable to complete evaluation in 24 hours, document cause for delay. Develop effective treatment plan, obtain approval, and treat patients per the physician plan. Supervise Speech Language Pathologist Assistants and other staff, communicating progress and plans. Participate in patient care conferences, utilization review, and rehabilitation conferences. Provide training programs for staff. Instruct families or nursing staff in maintenance programs and caregiver training. Act as a clinical preceptor for affiliating CFY/RFP SLPs and student programs.
Key Responsibilities
Perform evaluations, daily treatment notes, weekly progress notes, recertification documentation, and treatment changes.
Communicate with supervisors and health team members regarding patient progress.
Participate in discharge planning and in‑service training.
Act as a clinical preceptor and supervise CFY clinical fellows and restorative aides.
Qualifications
Bachelor's degree in Speech Language Therapy; preferred Master's or Doctorate.
Proficient in computer skills (e.g., Casamba, Point Click Care).
Valid Speech Therapist license in the state with good standing.
Continuous education requirements met.
Strong reading, comprehension, presentation, mathematical, and reasoning skills.
Physical Demands and Work Environment Essential functions require standing, walking, reaching, pushing/pulling, and occasional lifting >100 lbs. The work environment is low to moderate noise level. Reasonable accommodations may be made.
