The

Registered Physical Therapist (PT)

provides physician-prescribed physical therapy services to patients in their homes. The goal is to restore function, reduce or correct physical impairments, promote health, and teach skills for independent living. The PT works under the supervision of the

Director of Nursing

and collaborates with other healthcare professionals.

Locations:

Bay City (77404, 77414) & Wharton (77488, 77453, 77448, 77436) areas Salary:

$109,000 to $119,000 annually

Key Responsibilities Deliver physical therapy according to physician orders and care plans. Assess patient history, perform evaluations, and develop/revise care plans. Improve or minimize residual physical disabilities and help patients reach optimal functional levels. Instruct patients and families on home programs and daily living activities. Prepare clinical and progress notes; participate in case conferences and utilization reviews. Supervise Physical Therapist Assistants (PTAs) and Home Health Aides as needed. Maintain compliance with federal, state, and agency standards.

Requirements Experience:

Preferably 2 years in home/community physical therapy. Education:

Graduate from an accredited Physical Therapy program. Licensure:

Must hold a valid Physical Therapy license in Texas. Skills:

Strong communication (verbal and nonverbal), supervisory ability, independent judgment, and initiative. Physical Demands:

Ability to stand, walk, bend, lift, and transport equipment. Other:

Professional demeanor, ability to work in a busy environment, and adherence to agency dress code and ethical standards.

Work Environment Heavy interaction with patients, families, physicians, and third-party payers. It requires travel to patient homes and use of therapy-related equipment (e.g., stethoscope, BP cuff). Must maintain punctuality, professionalism, and compliance with agency policies.
