Essential Duties, Functions and Responsibilities
Provide diagnostic, preventative, and therapeutic health services to patients and family members and function as an independent practitioner or with appropriate physician supervision.
Secures a health and developmental history from the patient or parent and performs a detailed physical exam, records findings, and makes critical evaluations.
Performs or requests special screening, developmental, diagnostic tests, and procedures and interprets the results.
Makes decisions concerning medical care needs of patients which may involve various treatment modalities, prescribed medications, consultation and referral when appropriate.
Provides direct and independent medical care in the presence of illness or disability to maintain life, provide comfort, reduce distress, and enhance coping ability.
Provides surveillance of chronically ill persons; adjusts treatments within standing orders and recognizes when to refer the patient to a physician or team member.
Works collaboratively with physicians in the management of selected complex medical problems.
Participates with other professionals and agencies involved in providing health care, including immunization, disease control, environmental health, and health education.
Evaluates medical aspects of treatment plans periodically and recognizes when reassessment by a physician is appropriate.
Provides medical care and preventive services to healthy individuals, including guidance in nutrition, common illnesses, accidents, and immunizations.
Maintains knowledge and proficiency in medical practices through continuing education, staff meetings, workshops, and home study.
Teaches patients and family members ways to maintain or improve their health status.
Maintains records and reports on patients, activities, and other required information for program operation, quality assurance, and other purposes.
Participates in Quality Assurance (QA) and Performance Improvement (PI) review processes.
Provides full range of emergency services or crisis intervention, including lifesaving emergency procedures until a higher level of care is available.
Serves as a mentor to Advanced Practice Clinician I as needed and assumes responsibility for medical professionals assigned.
Performs other duties as assigned.
Experience Minimum mandatory qualifications:
Two (2) years direct work experience in a hospital/clinic setting as a licensed Nurse Practitioner or Physician Assistant.
Education Nurse Practitioner
Completion of an Advanced Nurse Practitioner School in any of the following areas:
Accredited Family Nurse Practitioner
Accredited Adult Nurse Practitioner
Accredited Adult-Gerontology
Active unrestricted Advanced Nurse Practitioner license in any U.S. state or territory in any of the following areas:
Family Nurse Practitioner
Adult Nurse Practitioner
Adult-Gerontology
National Certification from the American Academy of Nurse Practitioners
Active unrestricted Nurse Practitioner license from any U.S. state or territory
Physician Assistant
Completion of an accredited Physician Assistant Program.
Board Certification by the National Commission on Certification of Physician Assistants.
Active, unrestricted Physician Assistant license from any U.S. State or Territory.
