The Case Manager/Commu provides emotional support, guidance, and services to families dealing with serious medical conditions. The Case Manager assesses clients and their families and develops comprehensive, individual solutions to meet their practical, emotional, and social needs by assessing their strengths and needs, plan a course of action, link the client to services, counsel the client and monitor progress. Must assure that clients receive appropriate, competent, and professional information and advice, referrals, and advocacy services to achieve successful outcomes all in an effort to bring comfort and support to families and joy and hope to their children. Additionally, the Case Manager will engage in community outreach, building relationships with key leaders and service providers, including Rabbis, doctors, and schools within the Sephardic community, to ensure comprehensive support for families in crisis.

RESPONSIBILITIES INCLUDE: Case Management

Determine clients’ eligibility for assistance

Collect information about clients, maintain and enter information into databases

Maintain case history records and prepare reports

Refer patients to appropriate health care, community or social services or resources both locally and nationally

Provide advocacy to client families as needed, including making phone calls, writing letters, or accompanying clients to meetings

Work with staff, both locally and nationally, to coordinate and implement Chai Lifeline services, including but not limited to food deliveries, family/client events, volunteers, Big Brothers/Sisters, Camp Simcha programs, Insurance Support, medically supervised trips, Family Weekend Retreats

Counsel and provide emotional support to clients and family members

Monitor clients to evaluate progress

Schedule and confirm appointments for clients

Coordinate communication between patients, family members, medical staff, administrative staff

Assist, attend, and help execute all local Chai Lifeline programs: Big Brothers/Sisters, Camp Simcha without Boarders, Respite, events, etc.

Research local and national organizations to identify resources to meet the specific needs of families

Organization will provide training in trauma response

Actively participate in local trauma team interventions and services

Community Liaison – New York Sephardic Community

Community Outreach developing relationships with the Community Leadership and Service Providers

Rabbis, Doctors, Principles, and Community stakeholders, etc. familiarize them with the many facets of the services provided by Chai Lifeline to a family in need and build a personal connection with them if one of their congregants, patients, or community members is faced with Crisis or the trauma of a seriously ill child

Establish a professional relationship with the varied Schools in the community to aid in facilitating the individualized services needed for a Chai Lifeline patient, siblings, and family

Identify and develop new ideas and opportunities for programs for the community and families

Assist in ad hoc assignments

Education:

Master’s level mental health professional (MSW, MFT) with experience in medical setting

Professional Skills:

Must be self-motivated and driven to help

Strong familiarity with the Sephardic community, culture, and institution

Proven experience working with children, youth, and families

Excellent written and oral communication skills

Ability to multitask, work independently and as part of a team

Creativity and initiative needed to develop new program ideas in response to needs

Nonprofit experience is a plus

Current driver’s license and car
