Your Role As an MDS Coordinator, you will work collaboratively with an interdisciplinary team to assess residents’ needs and optimize their care plans. Position is onsite.
Key Responsibilities
Facilitate the pre‑admission process, securing essential information for MDS optimization
Review resident care and set ARDs to ensure accurate Prospective Payment System MDS Assessments
Maintain state and federal compliance with timely and precise resident assessments
Accurately code MDS data using information from staff interviews, medical records, and resident observations
Oversee MDS schedules and assist with other assigned responsibilities
What We’re Looking For
Active RN License in Oregon (or ability to obtain)
Strong interpersonal communication and teamwork skills
Experience with the RAI process and Case Management
Willingness to travel frequently across Oregon
