Collaborates with healthcare teams to develop and coordinate comprehensive, patient-centered care and discharge plans. Conducts psychosocial assessments and provides emotional and behavioral support for patients and families. Connects patients with community resources and ensures timely referrals based on identified needs. Maintains accurate documentation and communicates effectively with caregivers, case managers, and external agencies. Adheres to regulations and safety standards while participating in quality improvement, education, and outreach efforts. Qualifications
Master’s degree in social work from a school accredited by the Council on Social Work (Required) 1–2 years of experience in social work in a hospital setting (Preferred) 1–2 years of experience in care/case management (Preferred) Licenses / Certifications
LMSW – Licensed Master Social Worker in DC or Maryland, as required by location. LGSW – Licensed Graduate Social Worker in DC or Maryland, as required by location. CCM – Certified Case Manager (Preferred) Compensation
USD $68,036.00 – $111,259.00 per year.
