Medical, Dental, Vision, time off, paid holidays off, etc. Job Summary: The Correctional Case Manager is responsible for delivering confidential, client-centered services to individuals living with HIV/AIDS, both within correctional settings and the broader population. This role supports clients in accessing and adhering to healthcare, psychosocial, and supportive services, particularly those facing challenges in maintaining consistent care. The Case Manager will collaborate closely with multiple teams to stabilize clients medically and ensure ongoing engagement with primary care services. They will also provide adherence counseling for complex HIV treatment regimens, facilitate referrals to core services (e.g., dental, mental health, substance abuse), and assist clients in navigating public benefit systems. This position involves outreach, group facilitation, training support for new staff, and active participation in case management assessments for closed clients. Primary Responsibilities: Client Intake and Assessment Conduct comprehensive intake assessments for new and existing clients, including CBC intake and consent forms every six months. Initiate referrals to behavioral health, health education, dental care, and benefits programs. Evaluate eligibility and assist with applications for benefits such as MAP/ADAP, Medicaid, and other assistance programs. Service Planning and Goal Setting Develop individualized service plans addressing post-incarceration needs and other barriers to care. Collaborate with clients to establish achievable goals with clear outcomes and timelines. Reassess goals at least biannually and update plans in coordination with medical providers. Care Coordination and Adherence Support Conduct appointment reminders and maintain regular contact with clients (minimum quarterly face-to-face visits). Educate clients on clinic visit expectations, treatment adherence, and the importance of follow-through on referrals. Identify and address barriers to care (e.g., transportation) and re-engage non-compliant clients through outreach efforts. Referral Management Make timely and appropriate referrals to internal and external providers, documenting and following up to ensure service delivery. Collaboration with Medical Providers Participate in client medical appointments to advocate for social service needs. Attend clinic team meetings and complete care conferences with licensed providers biannually. Transition and Closure Facilitate appropriate program transitions, including referrals to DRS or other agencies as needed. Complete all required change-of-status documentation and support client participation in support groups. Documentation and Compliance Maintain accurate records of all client interactions, referrals, and services in required databases (e.g., EMR, CareWare, Provide). Properly document use of client incentives (e.g., transportation, food vouchers, phones). Submit reports, mileage reimbursements, and productivity logs in accordance with policy. Community Resource Management Stay informed of community and internal resources that can support client reintegration and care retention. Maintain an up-to-date resource directory and understand public benefit programs such as MAP, ADAP, Medicaid, Medicare, and ACA. Training and Education Participate in mandatory trainings (e.g., AFC Case Management Certification within 6 months). Provide training support to new case managers as needed. Attend workshops to enhance professional knowledge and specialization. Quality Improvement and Team Participation Engage in continuous quality improvement initiatives and submit required reports to funders and administration. Adhere to all company and social services protocols and policies. Provide clinic and coverage support when needed and take on additional duties as assigned. Qualifications: Experience: Prior experience in hospital or outpatient settings preferred. Experience with HIV-positive and/or correctional populations strongly preferred. Familiarity with case management, client support, and benefits navigation is essential. Skills: Strong written and verbal communication. Proficient in Microsoft Office, Access, and similar client databases. Ability to manage high caseloads in a fast-paced environment. Bilingual abilities preferred Other Requirements: Understanding of HIV/AIDS issues and a culturally competent, ethical, and confidential approach to client care.
