For over 125 years, Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens, currently offering 160+ programs and services for children, youth, adults, seniors, and those struggling with mental illness. Catholic Charities provides comprehensive care coordination and treatment services to individuals living with serious mental illness, complex medical needs, and substance use needs. Under the NYC Department of Health and Mental Hygiene, our Non-Medicaid Care Coordination Program works with individuals who do not qualify for Medicaid and are living with serious mental illness. We deliver community-based services to ensure clients have access to uninterrupted and coordinated behavioral and physical health services while addressing social determinants of health impacting their daily lives. Care Coordinators address issues such as housing, access to nutritious food, economic security/benefits, medication adherence, linkage with outpatient treatment providers, and other community resources and social supports. Responsibilities

The Care Coordinator is responsible for coordinating all aspects of an individual's care with complex and/or psychiatric co-morbid conditions, facilitating access to medical and psychosocial services efficiently and effectively. Care is provided in clients' homes/community at least twice monthly, or more frequently if needed. Duties include: Integration and coordination of physical health, mental health, and social needs. Active participation in care transitions to ensure follow-up care and re-engagement of lost-to-care patients. Monitoring and tracking data electronically, alerting team members when follow-up is needed. Engaging and retaining individuals in care, supporting adherence to treatment, and monitoring needs including prevention, wellness, and social services. Developing, managing, and coordinating personalized, person-centered care plans with clients, families, and providers. Ensuring priority appointments within provider networks to avoid unnecessary emergency or inpatient services. Promoting wellness and prevention resources tailored to individual needs. Sharing client information across providers using electronic databases and monitoring outcomes. Reassessing service needs and reviewing clinical measurements. Identifying barriers to care and resolving them. Completing required documentation, maintaining accurate electronic case files. Utilizing technology and web-based platforms for documentation and daily activities, with proficiency in devices like iPhones, tablets, and reporting systems. Qualifications

Education/Certifications:

Bachelor's degree in social work, psychology, or related health/human services field with 2 years of relevant experience, or certification in Medical/Clinical Assistance or health professions. Experience and Skills:

Crisis intervention, time management, psychosocial rehabilitation, linking clients to community resources, excellent communication, cross-cultural competency, outreach, advocacy, negotiation, engagement, monitoring, clinical assessment, and knowledge of community medical resources. Proficiency in computer skills (MS Word, Excel, PowerPoint, Outlook). Bilingual abilities (Spanish, Russian, Creole, Cantonese) are preferred. Benefits
