Responsibilities:
Develop therapy treatment plans for home health patients
Implement standardized tests for accurate diagnosis of physical, cognitive, and communication disorders
Establish treatment goals and milestones with patients
Timely documentation of treatment plans and progress reports
Supervise PTAs and COTAs (for PTs and OTs)
Update and communicate patient progress to relevant parties
Research related physical and functional problems
Qualifications:
Current California Professional License required
At least 2 years of clinical experience as a therapist (including outside of home health)
Previous experience in home health is preferred but training is available
Current CPR certification
Valid driver’s license with insured vehicle in good condition
Skills:
Meet physical requirements for clinical duties
Ability to build rapport with patients
Excellent communication skills
Positive, patient demeanor
Strong organizational and leadership skills
Basic computer and typing skills
