Program Coordinator – ACT Teams and Health Home Services
Full-Time | Monday–Friday, 9:00 AM–5:00 PM | On-Call Rotation Required

Base Pay Range $95,000.00/yr - $100,000.00/yr
About the Role Are you passionate about making a real difference in the lives of individuals living with mental illness? CEC Health Care, a leading not-for-profit organization in Nassau County, is seeking a dedicated and compassionate Program Coordinator to join our Assertive Community Treatment (ACT) Team.
Responsibilities
Provide clinical and operational oversight to multiple ACT teams, ensuring compliance with OMH standards and fidelity to the ACT model.
Oversee Health Home program implementation, quality assurance, and performance improvement initiatives.
Conduct staff performance evaluations and support professional growth through coaching and mentorship.
Promote recovery-oriented, trauma-informed, and culturally responsive care.
Facilitate integrated care planning that incorporates behavioral, medical, and social service coordination.
Prepare and submit reports, documentation, and data to regulatory agencies and internal stakeholders.
Establish and maintain collaborative relationships with hospitals, primary care providers, housing agencies, and community-based organizations.
Participate in after-hours on-call rotation as assigned.
Qualifications
Master’s Degree in Social Work or Mental Health Counseling.
Current New York State Licensed Clinical Social Worker (LCSW) or Licensed Mental Health Counselor (LMHC).
Minimum of three (3) years of progressive leadership experience in behavioral health settings.
Prior experience working in an ACT program strongly preferred.
Clean and valid NYS driver’s license and ability to travel throughout Nassau County.
