Responsibilities The
Physical Therapist (PT, DPT, RPT)
in Home Health assesses and evaluates patient care needs related to functional status, mobility, endurance, pain, and other medical conditions. Based on this assessment, the therapist develops and implements treatment plans, performs interventions to improve patient well-being, and monitors progress.
Evaluates the patient's functional status and therapy needs, assisting physicians in developing the care plan. Observes and reports patient responses and condition changes to the supervising nurse and/or physician. Treats patients to relieve pain, restore or develop function, and maintain maximum functioning according to accepted standards. Instructs patients, families, and healthcare team members about the care plan, limitations, and progress.
Education and Experience Licensure Requirements: Current Physical Therapy license in MA Current CPR certification Valid driver's license, vehicle insurance, and access to a reliable vehicle
