Position Summary As an Independent Optometrist leasing space inside Sam’s Club, you will lead patient care in eye and vision care as well as general health and well‑being. You will have autonomy to set practice days, hours, patient schedule, fee schedule and keep 100% of exam/professional fees.

Provide comprehensive eye and vision examinations and treatments
Manage patient schedule and control appointment flow
Set fee schedule and collect exam/professional fees
Maintain high standards of patient care and compliance with state regulations
Qualifications
Doctor of Optometry (OD) from an accredited optometry school
Valid state optometry license
General liability certificate
Professional liability insurance certificate
W‑9 form
Optional Equipment/Support Options
Equipment/instrumentation
Administrative and/or opt‑tech support services (if state allows)
Minimum Qualifications for Lease Agreement
Doctor of Optometry (OD) from accredited school
Valid state optometry license
Ophthalmologist MD or DO from accredited school
Valid state ophthalmology license
General liability certificate
Professional liability insurance certificate
