Responsibilities

Practice as a clinical pharmacist generalist in one or more patient care areas.

Provide patient-centered, interdisciplinary pharmaceutical care and proactively review and process orders accurately and efficiently.

Prevent and resolve drug-related problems by consulting with the ordering prescriber and documenting thoroughly.

Perform daily in-depth patient profile review, assess, monitor, recommend appropriate drug therapy, and evaluate, intervene, and document clinical interventions according to institutional protocols and guidelines.

Utilize system nomograms and other guidelines to monitor drug therapy regimens and make recommendations or modifications per protocol.

Engage in complex clinical situations with other clinical pharmacists and leadership as needed.

Participate in coordination of transitions of care, transitions of medication therapy management, and patient satisfaction.

Maintain clinical competency, oversee work of pharmacy students, technicians and pharmacists, and support cost-savings initiatives.

Perform or participate in medication use evaluation or quality improvement projects annually.

Qualifications

One year of relevant experience; Pharmacy Specialty Board certification or ASHP- Accredited Post Graduate Year 1 pharmacy residency preferred.

Active Georgia Pharmacist license.

Basic Life Support (B) Certification within 90 days; Advanced Cardiac Life Support (ACLS) required within 6 months if directly applicable.

Ability to lift 50 lbs max; hand and foot coordination; computer and typing proficiency.

Physical Requirements Medically capable of lifting 20-50 lbs, 0-33% of the work day occasionally; 11-25 lbs, 34-66% frequently; 01-10 lbs, 67-100% constantly. Carry objects up to 25 lbs. Occasional to frequent standing, walking, sitting. Close eye work. Environmental exposures vary.

Environmental Factors Blood-borne pathogen exposure; biohazardous waste; chemicals/gases/fumes/vapors; communicable diseases; electrical shock; floor surfaces; hot/cold temperatures; indoor/outdoor conditions; latex; lighting; patient care injuries; radiation; shift work; travel required. Personal protective equipment and respirators as appropriate.
