A growing healthcare organization in Crown Heights is seeking a
Service Coordinator
to manage a caseload of up to 25 patients. This role involves arranging transportation, ordering medical supplies, completing paperwork, and ensuring that all monthly and yearly documentation requirements are met. The ideal candidate will have strong case management skills, patient retention abilities, and a background in de-escalation. You will also conduct quarterly in-person visits with patients to maintain strong relationships and provide support. Responsibilities Include: Manage case files for up to 25 patients. Coordinate transportation, order medical supplies, and handle necessary paperwork. Ensure monthly and yearly documentation is completed accurately and on time. Conduct quarterly in-person visits with patients to assess needs and maintain relationships. Apply patient retention strategies and handle de-escalation when necessary. Maintain regular office hours (9-5) and observe. Ideal Qualifications: Previous experience in case management or patient coordination. Strong organizational and multitasking skills. Excellent interpersonal and communication abilities. Ability to handle patient concerns and resolve issues efficiently. Detail-oriented with experience in managing documentation and reports.
