Centra Health Job Description Provide patient care under the supervision of a licensed medical practitioner, registered nurse, or other licensed health professional, as authorized by the Health Regulatory Boards. The LPN coordinates and manages care for patients of all ages based on the unit care delivery model, requiring professional knowledge and consideration of specific patient conditions and treatments. This role collaborates with the interdisciplinary team to deliver high-quality patient care, adheres to Centra’s best practices for quality and service, maintains a safe patient environment, and coordinates care processes to achieve desired outcomes. The practice of the LPN aligns with the American Nurse Association’s Code of Ethics, the Nurse Practice Act of Virginia, and the ANA Principles of Nurse Staffing. Responsibilities Clinical - Assessment
Conduct initial assessment in collaboration with RN for co-signature. Perform ongoing assessments based on patient condition, policies, and RN guidance; adjust care accordingly. Integrate nursing interventions into the multidisciplinary care plan, following standards of care to achieve patient outcomes. Clinical - Intervention
Administer treatments ordered by providers accurately and promptly. Perform therapeutic interventions per individualized care plans. Recognize urgent issues and respond appropriately, including initiating emergency measures if needed. Provide culturally sensitive, non-judgmental care that respects patient autonomy and dignity. Collaborate with team members to ensure appropriate interventions. Administer medications following organizational policies. Clinical - Evaluation
Assess patient responses to care and evaluate effectiveness. Adjust care plans based on evaluations, collaborating with the team to improve outcomes. Document assessments, responses, and progress. Reinforce patient education. Clinical - Self and Peer Learning
Maintain current competencies; participate in educational activities and professional development. Resource Utilization and Stewardship
Support appropriate resource use. Serve as a preceptor/mentor, assisting in orientation and training of new staff and students. Quality Assurance and Safety Practices
Participate in quality data collection and initiatives. Identify and communicate practice concerns and participate in quality improvement efforts. Utilize safety behaviors consistently. Research/Evidence-Based Practice/Quality Improvement
Participate in research or quality improvement projects as assigned. Communicate findings and interpret data to peers. Ethics
Maintain confidentiality and protect patient rights and dignity. Advocate for patients and uphold professional boundaries. Other duties may be assigned.
Qualifications Education: Completion of an accredited LPN training program. Minimum Work Experience: At least one year of nursing experience; experience with medical equipment preferred. Preferred Experience: Prior experience in a similar setting. Licenses & Certifications: Valid Virginia LPN license, BLS certification. Seniority level
