Under the direction of the Medical Director, the Psychiatric Mental Health Nurse Practitioner collaborates with Physicians and the IDT team to provide assessment, planning, primary care, and ongoing medical education to participants. The role involves visiting the closest center in La Quinta, CA three times a week, with remote coverage for other locations listed below. Other Job Locations:
La Quinta, CA Riverside, CA San Jose, CA Essential Job Duties:
Provide psychiatric consultation, evaluation, counseling, and treatment for participants in collaboration with PACE Physicians and Community-Based PACE Physicians. Perform in-person or telehealth psychiatric assessments in collaboration with a physician and develop care plans with the IDT team. Assist in discharge planning from acute or long-term care settings in collaboration with physicians and the team. Refer participants to psychiatrists as indicated, in consultation with physicians. Oversee participant care in nursing homes, making regular visits based on standards and needs. Serve as a consultant to the IDT, communicating changes, collaborating on care plans, and coordinating 24-hour care delivery. Job Requirements:
Master's degree in nursing with a Psychiatric/Mental Health emphasis from an accredited institution. Certification as a Psychiatric Mental Health Nurse Practitioner. Active, unrestricted Psychiatric Mental Health Nurse Practitioner license in the state. State CDS Certification and DEA License. NPI license. Seniority level
