was founded in 2016 when SNFs shifted focus towards value-based care while treating residents with higher acuity. MDS PC emphasizes quality and customer service, prioritizing in-place treatment to reduce unnecessary hospitalizations. We have extensive experience providing medical services in SNF settings, understanding effective medical care delivery and the survey process. Currently, we serve facilities with specialty units, including residents on ventilators. Our providers actively communicate with surrounding hospitals to ensure continuity of care. We focus on reducing medication use, especially narcotics and psych meds, and ensure proper documentation for any necessary medications to avoid citations.
