What You'll Do Integrated Team Counselor - Mobile (ITC) within Adult Community Clinical Services (ACCS) teams provides clinical interventions and peer/family support to engage, support functioning, and help individuals stabilize symptoms and develop self‑management skills. The ITC works under the Team Leader and delivers services and interventions identified in individual Treatment Plans, including education, supervision, and direct support to help clients maintain physical and emotional health, live safely, and reach their goals.

Key Responsibilities

Provide input to person-centered assessment and service planning, advocate, coordinate, and link with other services and the community.

Provide rehabilitation interventions, including skills training in community environments.

Provide education, training, and physical intervention when needed to help persons learn life skills (budgeting, banking, transportation, housekeeping, personal hygiene, self‑medication, nutrition, health).

Model and teach skills for developing positive relationships and negotiating conflict.

Assist persons with accessing necessary health care.

Maintain regular clinical records (daily, monthly, annual) in accordance with DMH, agency, and division policies and within required timelines.

Consistently perform medication administration and documentation per Riverside and State MAP policies.

Schedule Full Time, 40 hours per week – Tuesday through Saturday.

Pay $21.37/hour

Required Skills

Demonstrated ability to work effectively in a team environment

Excellent written and verbal communication skills (English)

Valid driver's license and dependable transportation

Required Experience Preferred: Bachelor's Degree; Required: High School diploma.
