Family Medicine with OB/Family Med with CentraCare in Melrose, Minnesota
Join a thriving rural practice with 4 Family Medicine physicians and 6 Advanced Practice Providers providing a full spectrum of care in Central Minnesota.
4-day work week, 3 days in clinic and 1 day in the Level 4 Emergency Department
Additional ED shifts available for those interested, fewer ED shifts available if desired by provider
Weekday call is approximately 1:4, providing backup call to APP overnights. Weekend call with primary hospitalist duties including admissions and rounding. Opportunity to grow your OB practice while providing OB backup call to partners. Dedicated primary clinic nurse (CMA, LPN or RN) with support of float nursing. Co-located workstations for provider/nursing with an additional office. Opportunity for voice-recognition software (Dragon) or scribe. Team-based care including RN nurse lead visits, diabetic education, health care home for complex patient care, legal aide, health care navigator for uninsured/underinsured. Community health worker and community paramedic for patient engagement and home visits. Prescription support programs for uninsured/underinsured. In-room schedulers to promote continuity (follow-up appointments made for patients before they leave the room). CentraCare Foundation supported Reach Out & Read Program. Mental Health support including Integrated Behavioral Health with often same-day video consultation and Psychiatry chart review for medication management. E-consults for specialty care to allow for PCP management without the wait for a formal consult. CentraCare Melrose- Hospital State-of-the-art Critical Access Hospital built in 2017 with significant provider and community input. Services include: Telestroke and STEMI activation with St. Cloud Hospital, Advanced imaging with 5 days/week MRI availability, 3D Mammography (2021), Nitrous oxide for obstetrical and ED patients, Lactation consultants. Rehabilitation Services including Multidisciplinary Cardiac Rehabilitation and Pulmonary Rehabilitation. Transitional Care Team including a nursing supervisor, PT, OT, speech therapy and social worker. Outpatient Services including chemotherapy and IV infusions of fluids and medications.
