Description The

Low Threshold Case Manager

is responsible for coordinating and delivering case management services to individuals at our Low Threshold Housing Program. This program provides shelter, case management, housing advocacy, navigation, and recovery referral services to individuals who are unsheltered or homeless with chronic substance use. Our goal is to help individuals address barriers to housing, support those seeking to reduce substance use, and find permanent housing. The

Low Threshold Case Manager

reports to the Program Director for support and supervision.

Essential Functions

Act in accordance with Victory Programs, Inc.’s Standards of Excellence & Philosophy Care, maintaining professional ethics, boundaries, and client confidentiality.

Provide supported navigation, weekly individual case management, and appointment accompaniment for clients seeking housing, recovery, health, or social services.

Participate in developing and facilitating groups and workshops addressing barriers to housing and other assessed needs.

Engage in relationship building, Nonviolent Crisis Intervention, Motivational Interviewing, and conflict de-escalation as needed.

Maintain accurate and timely client records, including assessments, case notes, referrals, goals, and discharge documentation.

Advocate for residents facing behavioral health, medical, benefits, and legal barriers to housing.

Maintain a current database of referral sources such as detox, outpatient counseling, and self-help groups.

Make appropriate referrals to addiction treatment programs and monitor progress.

Work with the VPI Mobile Prevention Team to co-facilitate education and intervention groups related to housing, substance use, HIV/AIDS, and STIs.

Develop and implement aftercare plans for housed clients in collaboration with the team.

Ensure accurate documentation in electronic health records and program logs.

Complete all reporting requirements.

Maintain effective communication and positive relationships with team members and partners.

Enforce policies to maintain a safe, healthy, and inclusive residential environment.

Attend supervision, staff meetings, and professional development sessions.

Promote community participation and distribute information about local resources.

Non-Essential Functions

Participate in agency events, advocacy, and fundraising activities.

Perform additional duties as assigned by supervisors.

Required Qualifications

At least 2 years’ experience in substance use field.

Experience working with homeless populations.

Knowledge of HIV/AIDS, hepatitis, homelessness, substance use, STIs, and related social issues.

Ability to work effectively with diverse populations.

Excellent verbal communication skills, including discussing sex and substance use risks.

Compassionate, helpful, and directive approach to residents.

Proficiency in Microsoft Word and Excel.

CPR certification (may be obtained upon hire).

TB screening (may be obtained upon hire).

Valid Massachusetts driver’s license with safe driving points (00, 98, 99, 01, 02, or 03).

Preferred Qualifications

Bachelor’s degree or two years of case management experience.

Experience with poly-addicted individuals.

Background in human services or harm reduction.

Strong organizational and communication skills.

Ability to work independently and in teams.

Three professional references, including at least two from supervisors.

Bilingual skills.

Position Details Full-time, 40 hours/week, Monday to Friday, 9am-5pm, on-site.

Rate: $26.00/hour
