We offer a range of home‑based services that focus on practical, emotional, and spiritual care to maximize quality of life for patients, caregivers, and families. Position Summary : This full‑time Licensed Vocational Nurse (LVN) will work under the supervision of a Registered Nurse to deliver skilled patient care as outlined in the written plan of care. The role requires bilingual proficiency in Cantonese. Schedule : Tuesday, Wednesday & Thursday: 4:00 pm – 12:30 am Saturday & Sunday: 2:00 pm – 10:30 pm Location : San Francisco County and Marin County as needed. Essential Duties & Responsibilities : Assist a registered nurse in providing direct care. Initiate communication with physicians, managers, IPG members, HHA’s and other healthcare providers to facilitate optimal care. Instruct primary caregivers, patients, volunteers and employed caregivers in care provision based on the Plan of Care and as requested by the Primary Nurse or RN Manager. Attend patient conferences and participate in discussions of issues to facilitate optimal care. Education / Experience : Completion of a Licensed Vocational Nursing Education Program from an accredited school. Minimum one year of LVN experience, preferably in an acute care setting; experience in a home health/hospice setting preferred but not required. Certificates, Licenses, Registrations : Current Licensed Vocational Nurse certification from the California Board of Vocational Nursing and Psychiatric Technician Examiners.
