RN, Case Manager (Field based position servicing Fremont, CA and surrounding areas) Feel the value of excellence by providing quality medical case management for injured workers, coordinating appropriate care, making recommendations, and communicating with insurance carriers, healthcare providers, ancillary vendors, employers, and attorneys. This field-based role involves driving to and attending medical appointments in person, completing thorough documentation, and qualifying for bonus compensation. You have the flexibility to adjust your schedule and impact your earnings.
Compensation Range:
$120,000.00 to $180,000.00 annually
Essential Job Functions
Frequent travel within designated service areas to attend in‑person appointments and conduct case visits; potential overnight or air travel.
Manage assigned cases, including rush/same‑day referrals and attendance at medical appointments during facility hours.
Autonomous management of caseload, coordination of daily work activities and schedule.
Conduct on‑site case management services following CMSA guidelines and ISYS protocols.
Coordinate, facilitate, and complete in‑person attendance at physician appointments, hospital visits, case conferences, home visits, and ancillary provider visits.
Initiate professional discussions with medical providers regarding diagnosis, treatment, and prognosis.
Apply case‑management processes to facilitate timely treatment and recovery with goal of maximal recovery and return to work.
Provide proactive communication and coordinate collaboration with all parties in person, by phone, and/or writing.
Support injured worker’s progress toward Maximum Medical Improvement and realistic Return to Work options.
Facilitate timely claim resolution through research, analysis, and providing advice, direction, and action plans.
