This position is a clinically focused pharmacy position to provide pharmaceutical care to patients, provide consult services to physicians, support the research and educational objectives, and serve as informational resource to colleagues.
Responsibilities
Provide pharmaceutical care to patients including patient assessments, identifying and documenting drug therapy problems, resolving drug problems and providing drug information.
Coordinate pharmacy services with pharmacists and technicians.
Participate in departmental initiatives, including formulary and cost‑reduction initiatives.
Develop pharmacotherapy expertise for a select patient group; serve as a resource for the care of these patients.
Foster an environment of departmental teamwork with other pharmacists, technicians, residents, and students.
Develop and maintain interdepartmental relationships with physicians, nurses, and other healthcare professionals.
Serve as primary preceptor for the residency program and clerkship students from the College of Pharmacy.
Provide drug information for medical staff, nursing staff and patients.
Participate in performance improvement and medication error reduction initiatives.
Participate in team meetings.
Consistently demonstrates behaviors that fit with the Fairview mission, vision and values of dignity, integrity, service, and compassion. Recognizes appropriate values and behaviors in self and others.
Demonstrates respect for others and conducts self in calm, purposeful and professional manner.
Complies with all Fairview and departmental policies and procedures.

Respects the confidentiality of all information by sharing information only with those who need to know.
Understands and follows safety procedures specifically related to job duties. Demonstrates functional knowledge of emergency plans and utilizes proper infection control techniques.
Is punctual and complies with department standards regarding attendance.
Uses time effectively and actively participates in productivity of department.
Identifies and meets needs of customers.
Continuously works to improve skills and interpersonal relationships.
Required Qualifications
B.S./B.A. Bachelor’s degree in Pharmacy
Active Minnesota Pharmacist License
Minimum 3 years SOT experience
Preferred Qualifications
