Position:

Full‑time Vietnamese speaking Case Manager I in Aging and Adult Services. Provides culturally competent case management services to elderly persons and adults living with disabilities within the Asian and Native Hawaiian/Pacific Islander community. Services are as holistic, person‑centered, and strengths‑based as possible, with the goal of helping individuals to live a meaningful and independent life in the community.

COMPETITIVE BENEFITS PACKAGE INCLUDES:

Pay range: $58,400 to $63,100 annually. Pay can increase based on experience.

ACRS pays 100% for employee‑only HMO/Virtual Plus medical benefits. Additional options are covered from 75% to 85%.

Vision insurance is available at a very minimal cost.

ACRS pays 100% of life, AD&D, and long‑term disability insurance on or coinciding with start date.

Paid Time Off:

ESSENTIAL JOB FUNCTIONS:

Work with clients to develop and implement person‑centered, holistic care plans, conduct comprehensive in‑home functional assessments, and collaborate with client/family and community resources.

Identify client needs and facilitate access to community resources, manage referrals, and ensure services meet client needs.

Authorize services and client choice of qualified providers, travel for home visits, and maintain communication for implementation.

Authorize or re‑authorize Medicaid‑funded services via Provider One and inform clients of changes.

Advocate for services in citizenship, housing, food assistance, legal, translation, and other areas; manage LTC-related referrals and documentation.

Facilitate transitions to nursing homes, assisted living, or adult family homes; coordinate placement and continuity of care.

Report as a mandatory APS reporter and document suicidal ideation or behavioral issues.

Assist clients/families with home modifications, crisis intervention, and appropriate care setting transfers.

Complete documentation on time, participate in QA and process improvements, and meet performance standards.

Attend agency, program, and team meetings, consultations, and in‑service training.

POSITION QUALIFICATIONS:

Education and Experience:

Master’s degree (social work, behavioral, or health sciences) with bilingual/bicultural skills.

Bachelor’s degree (social work, behavioral, or health sciences) with bilingual/bicultural skills.

Associate degree plus one (1) year of paid on‑the‑job social services experience with bilingual/bicultural skills.

High school diploma plus three (3) years of paid on‑the‑job social services experience with bilingual/bicultural skills.

Completion of a case manager training program (e.g., State‑sponsored CORE Training) is beneficial.

Cultural Humility:

Communicate effectively with individuals from diverse backgrounds.

Provide respectful, responsive care aligning with cultural beliefs and practices.

Fluency in an Asian or Native Hawaiian/Pacific Islander language preferred; multilingual skills highly valued.

Knowledge and Skills:

Strong customer‑service skills with empathy, compassion, and patience.

Time‑management, problem‑solving, and critical‑thinking abilities.

Conflict‑resolution skills to mediate between clients and caregivers.

Knowledge of the aging process, aging network services, community service systems, and CLC & Title XIX Case Management programs.

Understanding of common adult illness progression and daily impact.

Judgment regarding client safety during assessments.

Technical Skills:

Proficient in Microsoft Office, Outlook, Teams, Zoom, and related web applications.

Research and internet‑search skills.

Additional Requirements:

Reliable transportation; if using personal or agency vehicle, valid driver’s license and adequate insurance required.

Follow all agency policies and the Employee Handbook.

WORKING CONDITIONS:

Requires regular mobility within the office or workspace, extended sitting, handling objects, and occasional lifting of up to 20 pounds. The position is hybrid, with about 60 % onsite (home visits) and 40 % remote. Noise level is moderate.
