In Clinic Visit Facilitator
Fairview is hiring an In Clinic Visit Facilitator for our Sleep Clinic team at the UMMC West Bank clinic in Minneapolis, MN. This is a 1.0 FTE (80 hours per two‑week pay period) position. Clinic hours are Monday‑Friday, 7:00 am – 3:30 pm (no weekends). The role is benefits‑eligible.
Responsibilities
Prepare patients for their appointments, including communicating in advance to ensure medical records are collected and available.
Assist in the rooming process by collecting and documenting pertinent information such as reason for visit, medication review, vitals, and health questionnaires.
Prepare exam rooms and maintain supplies.
Support patient connection to the provider and serve as a resource to ensure a positive patient/provider experience.
Complete visit wrap‑up, providing after‑visit summaries and ensuring patients are aware of follow‑up recommendations, including scheduling if needed.
Communicate effectively with patients, providers, and other health professionals, both in person and via virtual technologies.
Required Qualifications
High School Diploma or GED.
At least 1 year of customer service experience.
Proficiency with technology (computer hardware and software, smart devices such as phones or tablets).
Preferred Qualifications
Current student of nursing, medical, social work, psychology, or pharmacy.
Completion of Nursing Assistant, Emergency Medical Technician, or Health Unit Coordinator program.
Knowledge of medical terminology.
Experience with Epic Health Records.
Telephonic customer experience in a healthcare setting.
Previous work in a healthcare setting.
