is seeking a registered nurse to coordinate our substance use disorder (SUD) care program at the
Harbor Community Health Center in Hyannis, MA .
Location:
Hyannis, MA (Harbor Community Health Center)
Employment Type:
Full‑time, 40 hours per week
Schedule:
8:00 AM – 4:30 PM, Monday‑Friday (subject to change)
Salary:
$33.03–$57.07 per hour
Sign‑On Bonus:
$5,000 (half on hire, remainder at 90 days)
Position Summary:
The CCSRN SUD Care Program Coordinator is a Complex Care Registered Nurse responsible for delivering high‑quality, patient‑centered care to individuals with substance use disorders. The role involves 75 % direct patient care and 25 % administrative duties, working closely with an interdisciplinary team.
Responsibilities
Conduct initial assessments and intakes, gathering comprehensive histories.
Triage low‑barrier phone and walk‑in inquiries for treatment.
Review assessments with the SUD clinical team to determine care plans.
Manage patient care plans, perform crisis intervention, and coordinate referrals.
Collaborate with the SUD team and attend required meetings.
Provide ongoing education, support, and service coordination for patients.
Liaise with primary care and community partners (hospitals, labs, outreach teams).
Create clinical forms and ensure meticulous documentation.
Collect data and specimens, review results, and take clinical actions per protocol.
Administer immunizations as requested and document in EMR.
Coordinate with pharmacy on medication orders, administration, prescriptions, and refills.
Counsel patients on relapse prevention, pharmacotherapy, and resources.
Provide follow‑up care, schedule visits, and phone support.
Engage in primary care and addiction‑related projects.
Submit required data to the Bureau of Substance Abuse Services.
Assist with phone triage, prescription refills, test results, and medical issue management.
Assist providers with procedures, injections, EKGs, referrals, prior authorizations, specialty pharmacy, and controlled‑substance documentation.
Perform all nursing duties in accordance with Massachusetts Nursing Practice Act and Harbor Health policies.
Follow HHSI policy for biohazardous material disposal.
Administrative Responsibilities
Collaborate with nursing leadership and the Director of SUD Care on compliance, policy development, documentation, and outcome monitoring.
Participate in program planning and quality improvement initiatives.
Coordinate patient referrals to internal and external providers.
Support training and education for team members on evidence‑based care standards.
Maintain accurate patient records in the EHR.
Attend team meetings, case conferences, and community outreach events.
Requirements
RN license in Massachusetts (BSN preferred).
Certification as Certified Addictions Registered Nurse (CARN) or willing to obtain within 6 months.
Current BLS certification.
Minimum 2 + years clinical experience in SUD treatment, behavioral health, or addiction medicine.
Strong leadership, critical thinking, problem‑solving, and administrative skills.
Experience with medication‑assisted treatment (MAT) programs and withdrawal management.
Experience in outpatient or community health settings with cultural sensitivity.
Comprehensive understanding of MAT protocols and withdrawal management.
Excellent communication and interpersonal skills.
Intermediate knowledge of Microsoft Office 365; EMR experience preferred.
Experience or training in de‑escalation preferred.
Knowledge of community resources for health, protective services, crisis intervention, and disability.
Willingness to train and orient new personnel.
Strong patient advocate and anti‑stigma change agent.
