The Director of Health Services (DHS) embodies Yoi Shigoto by leading the overall implementation of health and wellness services in the community. The DHS serves as a liaison with families, hospital personnel, physicians, community organizations, and other service agencies to provide quality care to MBK Senior Living residents. The DHS is responsible for hiring, training, managing, and leading a care team to provide superior care while maintaining residents’ physical, psychological and emotional wellness, in accordance with MBK’s core values. Residents’ independence, dignity, autonomy, and purpose are integrated into the plan of care as written and directed by the DHS. Residents and their loved ones are partners in developing individualized service plans. Essential Job Duties Conduct and coordinate potential resident assessments in accordance with current rules, regulations, and community policies, including: initial assisted living resident assessment prior to move-in, comprehensive care plan at move-in; invite memory care director to participate in assessments for memory care residents; consult with regional director of health and wellness and AFG nurse case manager for complex cases Draft initial service plans for new residents; review with responsible parties and Executive Director Establish service plans for each resident to promote independence and total whole-person wellness; evaluate residents through this lens Conduct routinely scheduled re-assessments, change of condition assessments, and produce revised service plans; obtain signatures per policy Draft/update individualized service plans as needed; ensure personalization of care delivery; ensure best possible care and programs Conduct interviews and observations of residents continually; delegate service plan processes to memory care director as appropriate Alert the Executive Director, family members and others of changes in residents’ health and the community’s ability to meet needs under licensure Ensure continuity of residents’ total wellness by coordinating health and wellness services with other resident services; work with all departments to maximize services Act as resource and care manager for residents and families; contact physicians for orders as needed Develop and maintain good working relationships and communication with family and service providers Direct and assign duties to memory care director as appropriate; collaborate with wellness programming and kitchen leadership to ensure robust program offerings and nutritional wellness Participate in CQI, coordinate weekly interdisciplinary rounds, and oversee health/wellness databases and systems (wanderguard, eMAR, eHR, electronic check-in, pager) Collaborate on safety practices, disaster preparedness, infection control, and elopement drills; involve regional director of wellness programming for family education Involve MBK Together in family education offerings as appropriate Non-Essential Job Duties Perform other duties as assigned; collaborate with AFG nurse case manager and regional director to ensure policy and regulatory compliance Maintain safe environment; comply with all regulations and company policies Demonstrate tact and teamwork; support MBK principles and core values Assist Executive Director with annual budget planning and financial goals Minimize waste and misuse of supplies and equipment Supervisory/Management Responsibilities Health Services Assistant: 1 to 6 employees Caregiver: 8 to 48 employees (shared with Memory Care Director and/or Director of Assisted Living) Medication Technician: 2 to 16 employees (shared with Memory Care Director and/or Director of Assisted Living) Minimum Job Requirements Certified RN, LPN, or LVN or 2 years of full-time college attendance Completed required specialized nursing coursework for certification as RN, LPN or LVN Maintain license with CEUs and timely renewal Prior related supervisory/management experience in a similar setting Current First Aid Certification Background clearances as required Health screening with negative TB test within last 6 months or within 7 days of employment Valid driver’s license and vehicle insurance as required by state Proficient in Microsoft Office; strong communication skills (spoken/written English) Basic math skills for personnel actions and budgeting Ability to make independent decisions and stay calm in stressful situations Professional demeanor with residents, families, and staff Maintain neat, organized work environment for safe care coordination Valid driver’s license in state of employment and reliable transportation for assessments Preferred Job Requirements Bachelor’s Degree in Nursing, Human Services, or Health Care field Physical Demands Move intermittently; ability to lift/carry up to 40+ pounds and push residents up to 250 pounds Ability to assist in moving residents when necessary Pay Rate :
