Outreach Case Manager/ Direct Care Specialist

Essential Job Functions I. Direct Care:
Case management of assigned caseload
Coordination and collaboration with youths, families, residential treatment providers, clinicians and other providers regarding treatment plan development, strategies for intervention and implementation
Implement treatment plan and treatment strategies
Outreach to child/family/collateral/clinical/residential contacts that comprise the youth’s team, i.e., school, court, home, residential programs and other community agencies, etc.
Encourage and assist youths with enhancing natural supports and developing community connections
Supervise youths in after school programs, extracurricular and other community based activities as needed
Assist youths in attending treatment and services at the Brien Center and other relevant agencies as stated in youth’s treatment plan
Participate in treatment meetings for youths on assigned caseload
Advocate for youths and youth’s voice and choice
Provide transportation to family visits, meetings, appointments, activities, etc. to facilitate implementation of treatment plan
II. Administrative Duties:
Complete all paperwork in a timely manner
Participate in all relevant staff meetings, supervision and required trainings
Update supervisor on collateral contacts on a regular basis
Complete all tasks assigned which impact the organized functioning of the program
Qualifications
High School or equivalency required; Bachelor’s in human services-related field strongly preferred
2 years experience working in case-management and/or with children/adolescents and families required
Knowledge of WRAP curriculum preferred
CPR, First Aid and MAP Certified, or obtain CPR, First Aid and MAP certification within first 3 months of employment
