We are currently seeking a compassionate, organized, and team-oriented individual to join our Home Health team as a Full‑Time or Part‑Time Physical Therapist (PT) in Half Moon Bay, CA.
Position Overview We are seeking an experienced, self‑driven PT to provide exceptional care throughout the region. As a PT, you will evaluate patients’ physical health, develop individualized treatment plans, and work to improve their mobility, strength, and overall quality of life. You will collaborate closely with our Clinical Director and Clinical Staff, ensuring the highest standards of care while providing support both remotely and independently in the field.

Responsibilities
Provide physical therapy services within the scope of care, following the patient’s individualized plan, in coordination with the interdisciplinary team.
Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to the physician, case manager, and/or director of nursing as needed.
Maintain accurate clinical records and submit documentation according to company policy using a company‑provided device.
Comply with all company policies and procedures to ensure high standards of care.
Requirements
Current Physical Therapist (PT) license to practice in California – required.
Experience in home health or hospice care – 1 year required.
Current CA driver’s license.
Able to travel within a 15–25 mile radius on a regular basis.
