About the Position The Case Manager II will provide case management services to patients and their families or caregivers, including assessment, planning, implementation of interventions, follow‑up, and coordination of services. The role also engages in community outreach, performs administrative tasks, and supports nursing practice within legal and institutional standards.

Pay Range Base pay range: $82,347.20/yr - $137,238.40/yr. Payment is based on experience and qualifications.

Responsibilities

Conduct assessments and develop care plans for patients and families.

Plan and implement interventions, ensuring follow‑up and coordination of services.

Engage in community outreach activities and build relationships with local resources.

Perform necessary administrative tasks related to case management.

Document patient interactions and care plans according to departmental policies.

Participate in performance improvement processes and shared leadership initiatives.

Communicate denials, high‑risk situations, or changes in level of care using accepted processes.

Navigate internal and external systems to secure payers and link patients to services.

Develop and implement discharge and transitional care plans in collaboration with the healthcare team.

Provide clinical reviews to third‑party payers and document authorization/denial status in STAR.

Identify and implement processes to improve customer service, reimbursement, and hospital performance.

Support organizational accountabilities in teamwork, communication, financial responsibility, safety, and continuous improvement.

Qualifications

Master’s Degree in a related field (required).

Minimum of 5 years of nursing experience, with at least 3 years in a pediatric setting (required).

Registered Nurse license (required).

Current certification in Case Management (preferred).

Location & Work Schedule District of Columbia – Washington, DC (Main Campus). Day shift, Monday‑Friday 8:30 am–5 pm.

Position Status: Regular (Full‑time).
