Physical Therapist / PT – Flagstar Rehab
Flagstar Nursing is seeking Physical Therapists for Skilled Nursing Facilities and is currently staffing PTs for Rehab Departments in Stamford and Norwalk.
Flagstar Rehab PTs work with a diverse, multi-disciplinary team to perform evaluations, deliver treatments, and educate patients and families to improve physical and cognitive function, helping patients regain daily independence.
Employment options include direct employment with our agency or independent contractor (we can assist with establishing a corporation free of charge).
Hourly pay rates: $42–$46 (W‑2) or $55–$57 (1099). Sick leave and paid orientation are provided. Pay is deposited weekly.
Job Duties
Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
Supervise PTAs daily.
Deliver quality therapy services to individuals in an SNF setting requiring care for TBI, CVA, stroke, Parkinson’s, multiple sclerosis, cancer, orthopedic surgery, and other debilitating diagnoses.
Monitor and document patient progress using innovative EMR software.

Current CT PT license and registration.
CPR certification.
EMR competence.
