Speech Therapist - Home Care
Reporting to the Rehabilitation Team Lead, the Speech Language Pathologist is responsible for comprehensive patient management including evaluation, diagnosis, intervention, and patient/family education for development and implementation of individualized care planning in the home setting. Opportunities include working directly in a multidisciplinary fashion, including specialty care groups such as Parkinson’s Disease, Covid Recovery and Post‑Acute Stroke care. The ideal candidate demonstrates strong clinical skills in Speech‑Language Pathology, with opportunity for clinical growth through education, in‑services, and care management groups. Responsibilities
Comprehensive patient management: evaluation, diagnosis, intervention.
Patient/family education for individualized care planning in the home setting.
Multidisciplinary collaboration with specialty care groups (Parkinson’s Disease, Covid Recovery, Post‑Acute Stroke care).
Participation in clinical growth through education, in‑services, and care management groups.
Provide flexible, family‑friendly care in a Home Care setting.
Minimum Qualifications
Education
Master’s Degree in Speech‑Language Pathology from an accredited institution.
Experience
At least one year prior Acute or Rehab experience is preferred.
Licensure and/or Certification
MA SLP license. BLS required within 30 days from hire.
Skills
Clinical expertise for patient population.
Ability to prioritize workload and respond to urgent needs.
Communication, assessment, critical thinking, organizational, problem‑solving skills.
Computer skills.
Maintain confidential information.
Ability to read, write and communicate in English.
Committed to Service Excellence and customer satisfaction.
