Executive Summary:

HRUCKUS is looking for experienced Outreach Specialists to provide temporary staff support services to the DC Department of Health.

Position Description:

This position is in the Department of Health (DC Health), Community Health Administration (CHA), Family Health Bureau (FHB), and Early Childhood Health Division (ECHD). The Community Health Administration (CHA) works to prevent the leading causes of death, protect and promote the health of mothers and children and eliminate racial and ethnic health disparities in health. CHA focuses on nutrition and physical activity promotion; cancer and chronic disease prevention and control; access to quality primary health care services; and the health of families across the lifespan. CHA’s approach targets the multiple factors that influence health through evidence-based programs, policies and systems change. The Early Childhood Health Division (ECHD) works to ensure that all children and families have access to a continuum of comprehensive, high quality early childhood programs and services that promote child well-being and school readiness and ensure that all children are healthy, ready to learn and have safe passage through the early years. ECHD programs include Help Me Grow DC, Smart from the Start, Healthy Steps, Children and Youth with Special Health Care Needs (CYSHCN), and Home Visiting programs, including SafeCare Augmented (SafeCare). SafeCare is an in‑home behavioral parenting program for parents and caregivers of children ages 0‑5 years that aims to promote positive parent‑child interactions, informed caregiver response to childhood illness and injury, and a safe home environment. The incumbent is responsible for planning and conducting weekly home visits for families enrolled in SafeCare, administering screenings and assessments, maintaining an assigned caseload and providing families with resources, information, and referrals aligned with family needs.

Position Responsibilities:

Responsible for coordinating, planning and implementing weekly home visits for families, ensuring timely and efficient service delivery while maintaining clear communication with clients to support family engagement and attainment of program goals.

Administers assessments and screenings in accordance with programmatic guidelines, and makes appropriate, timely referrals based on client-reported information and identified needs to ensure access to supportive services.

Maintains a designated home visiting caseload ensuring families are actively and meaningfully engaged throughout the duration of the program.

Provide resources, information, training and referrals to families in the areas of need or interest, or as required by Performance Standards.

Builds and maintains trusting and respectful relationships with families through ongoing engagement, cultural sensitivity, and consistent support to foster open communication and promote positive health outcomes for participating households.

Collaboratively supports program promotion, outreach, and recruitment activities to increase community and family awareness and engagement, with the goal of maintaining individual caseload and strengthening program’s visibility and impact within all target populations.

Regularly supports the development of innovative community engagement strategies to target populations to build trust, foster relationships, and generate enthusiasm for participation in home visiting services in the District of Columbia but most specifically in Wards 5, 7 and 8.

Collects and maintains accurate and up‑to‑date data in all relevant databases, ensuring timely, efficient and organized data management in compliance with program standards and reporting requirements.

Actively participates in mandatory meetings, assigned trainings and professional development opportunities to enhance program knowledge, stay current with industry best practices, and support continuous improvement in home visiting service delivery.

Maintains positive and professional demeanor at all times.

Position Requirements:

An Associate’s or Bachelor’s degree in public health, psychology, education, social services or related health field with course work in the areas of child development, family relations, and interpersonal skills OR

A minimum of two (2) years’ experience in the area of social services, education or public health, specifically related to child health, behavioral health, social services, and/or early childhood.

2-3 years of experience in community outreach, public health, or a related field.

Experience in community outreach and relationship building.

Knowledge of federal and local assistance resources.

Ability to work with a wide variety of community stakeholders.

Pay Range: $17.90 to $23.26 an hour

Work Duration: Until September 2027 with possibility for extension.
